NeuroPhysioSolutions Ltd is committed to protecting your personal information and this privacy document relates to our use of your personal information collected from you either in person, via telephone call or e-mail. Personal information relates to any information that is capable of identifying you. "We" relates to NeuroPhysioSolutions. We collect and process data due to our legal obligation and it is relevant, adequate and limited to what is necessary.

Information we collect & when.

Information is collected and processed when you telephone the clinic or one of our Physiotherapists to make an enquiry or appointment, when you e-mail us or visit the clinic in person or undertake a home visit.

At the point of enquiry or booking we may request:

  • Your name
  • Your date of Birth
  • Your address
  • Your telephone numbers, landline/mobile
  • Your email address

At your initial assessment we will ask for information regarding your previous health, current health and the condition you are seeking treatment for.

We will also ask for information regarding any activities you undertake, your hobbies, employment, in addition to any current medication.

We also record all findings of our physical examination.

How we use the information:

We use electronic and some paper documentation, the electronic information is hosted by a third party (Nookal.com). Their data storage is in Ireland by AWS and is encrypted and secure in compliance with International guidelines of GDPR.

All information electronically kept is held in a file that directly relates to your episode of care, this information is used for:

  • To provide a legal record of any treatment or advice we provide
  • To ensure continuity of care
  • To contact you in regard to your on-going treatment, including sending exercises via e-mail via a third party (Physitrak).
  • To contact you if new information or treatments become available that may be of benefit to you.
  • We may use your information for audit and quality purposes
  • We may also pass information, with your permission, to other Health and Care professions.

We will not pass your information for commercial purposes.

We take all reasonable steps to ensure that our information is kept up to date and rectified if necessary. It is your responsibility to inform us if any of your personal information changes.

How long we keep personal information for:

We have a legal obligation to retain records for 8 years after the conclusion of treatment.

If the records relate to a child or young person, these records must be kept until the patients 25th birthday or 8 years after their 18 th birthday.

How do we protect your information:

We take organisational and technical security measures to protect the information against unlawful processing or unauthorised disclosure. Our website contains links to external sites which hold their own privacy policies which you should check prior to submitting any personal information.

Your rights:

You are entitled to a copy of the personal information we hold about you and to have any discrepancies corrected, this can be undertaken in writing to the above address at the top of the policy.

Disclosure of your information:

With your permission we may pass your information to other medical professionals who may be involved in your care, this may include: Consultants, Gp's, Care staff, Multidisciplinary team members or other Health and Care professionals.

This information may be passed in the form of a written letter, which may be given to you, in which case it becomes your responsibility, and the protection of its contents is your responsibility.

If the information is passed electronically by e-mail, it will be password protected or will be sent via secure e-mail. We will take all reasonable precautions to transmit the information securely.

Photographs and Video:

We may use photographs and video footage to record accurately certain aspects of your movement and posture. It will also form an on-going record of treatment progress and change. These images are stored securely in I-Cloud in a file related to your episode of care, also photographs are attached to your electronic notes. With your permission they may be used for teaching purposes within the practice, or more widely within similar professional educational forums. If there is ever the wish to medically publish any of your images, you will be contacted in the future for your specific agreement.
